• New patient referrals are requested to provide the following information. Feel free to print the information sheets, complete, and bring them with you upon your first office visit. This will lessen "filling out insurance form time."
• Co-pays and deductibles are to be paid with each visit when applicable.
• Treatment pre-authorization may be required by your insurance carrier. Our staff will verify this information and share your plan's benefits as provided to us by your insurance carrier. Please be advised what your insurance carrier reports to us may not be accurate.
• Bills are submitted to your insurance carrier weekly. Monthly statements upon request can mailed to keep you updated on your account status and responsibility.
